ATTENTION: As of September 1, 2021 the State of Illinois is now requiring all Courts to use standardized court forms and no longer provide local forms for the same legal remedy (see Rule 10-101). The Lake County Circuit Clerk's Office and 19th Judicial Circuit will no longer be providing certain court forms in which a standardized version exists and have removed our versions from our website.
